Download a form and deliver the completed form to the Capital G Welcome Desk at 19 Reid Street in Hamilton, from 8:30am to 5:00pm on weekdays, and 8:30am to 4:00pm on Saturday.

You should bring a valid piece of photographic ID and an acceptable proof of address such as a telephone or electric light bill.
